The problem is not always that the tidal volume is large in absolute terms. The problem is that the tidal volume may be large relative to the amount of aerated lung available to receive it.
Driving pressure is calculated as plateau pressure minus PEEP.
It reflects the pressure required to deliver the tidal volume to the respiratory system.
