PEEP and Hemodynamics: When Better Oxygenation Reduces Cardiac Output

PEEP can improve oxygenation.

It can also worsen shock.

Both statements are true.

PEEP increases pressure in the thorax. This can recruit collapsed alveoli, improve functional residual capacity, reduce shunt, and improve oxygenation.

But the circulation lives in the thorax too.
