The Case of the Rising Plateau Pressure: Compliance, Resistance, or Auto-PEEP?

A rising ventilator pressure is not a diagnosis.

It is a clue.

The key is determining whether the problem is airway resistance, respiratory system compliance, or dynamic hyperinflation.

Peak pressure is the maximum pressure during inspiration.

Plateau pressure is measured during an inspiratory hold when airflow stops. It better reflects alveolar pressure and respiratory system compliance.
